What we do is...
One person says a card name (make one up, don't use a real card)
The next player makes up a card that has that name.
If someone said, DARK DOOMLORD
I could make up this card:
Dark Doomlord
****
DARK
Fiend
ATK=1400
DEF=1000
This card gains 600 attack if there is another DARK monster on the field.
Get it?
So... someone make up this card...
Gishki paradise
(Remember: It can be a monster, spell, trap, fusion, ritual, etc. Do not make them all too overpowered!)
Then the player who designs the card "Gishki paradise" can say another name.

Field Spell
Effect: Each time a "Gishki" monster is normal summoned or special summoned place 2 counters on this card. Once per turn, during your main phase, you can Ritual summon 1 "Gishki" Ritual monster from your hand by removing counters from this card equal to the current level of that Ritual monster.
My card: Infernity Universe
Effect: Each time a "Gishki" monster is normal summoned or special summoned place 2 counters on this card. Once per turn, during your main phase, you can Ritual summon 1 "Gishki" Ritual monster from your hand by removing counters from this card equal to the current level of that Ritual monster.

Perma Trap

Continuous Trap Card
Effect: Once per turn, during your main phase, you can tribute 1 Dinosaur type monster you control then special summon 1 Dinosaur type monster from your deck too your side of the field face-up that is 2 levels higher then the tribute monster.
